Red Flags by Age
By 12 months — your child should be babbling, using gestures like pointing and waving, and reacting to their own name. A flag: no babbling, no gestures, and no response to sound.
By 18 months — expect around 10–20 words and a preference for words over gestures to communicate. A flag: relying only on pointing or grunting, and not following simple directions like “give me the ball.”
By 24 months — a vocabulary of 50+ words and simple two-word combinations. A flag: fewer than 50 words, no word combinations, or speech that even family can’t understand.
By 3 years — short sentences, questions, and being understood by people outside the family most of the time. A flag: strangers rarely understand your child, or they still speak mostly in single words.
Beyond Words: Other Signs to Watch
- Frequent frustration or meltdowns tied to not being understood
- Difficulty following directions or answering simple questions
- Not making eye contact or engaging in back-and-forth play
- Trouble with eating textures or excessive drooling (which can signal oral-motor concerns)
- Losing words or skills your child previously had — this one always warrants a prompt evaluation
